Are you getting this on the first save or the subsequent save?

If it is on a subsequent save..... I think what your code is not taking in consider that technically when you are resaving the document the formula code is thinking there are duplicates.

I would not make the field editable after it was created -- this would simply the code, but of course you can not edit the date field after you done the first save.
